Welcome to the Linwood School District!
We are excited your student will be joining us for their education journey.

All new Linwood Public Schools and all Kindergarten Students, please read the following information before proceeding:

**If your child has already attended the Linwood School District you DO NOT need to complete new student registration. This includes preschool and Kindergarten.
In order to complete the registration for your child(ren) we will need you to have the items outlined in the registration platform. Registration is incomplete until we have received the following items:

ALL STUDENTS

• Birth Certificate
• Immunization Record
• Physical(dated within the past year)
• Transfer Card (if transferring from another school within the State)

HOMEOWNER or LEASE/RENTER

• Deed/Mortgage Document or Tax Bill in the Parent's/Guardian's name
• Lease or Rental Agreement
• Utility Bill
• Proof of Identification

AFFIDAVIT (if Parent/Guardian lives with a Linwood homeowner):

• Affidavit signed by Homeowner with Notary Stamp and Signature
• Deed/Mortgage Document or Tax Bill in Homeowner's Name
• Utility Bill in Homeowner's Name
• Utility Bill in Parent’s Name

AFFIDAVIT (If Parent/Guardian lives with a Linwood renter)

• Affidavit signed by Renter with Notary Stamp and Signature
• Copy of Lease/Rental Agreement in Renter's Name
• Utility Bill in Renter's Name Utility Bill in Parent’s Name
• Proof of Identification
